The Healthy Choice program is a way for Health Plan members to take charge of their well-being.  Caregivers and their spouses who participate can improve their health and get up to 30% off their premiums by meeting personalized annual medical, nutrition or fitness goals.  Each year, more members are getting healthier and achieving larger discounts.  Join in on the benefits!

Important Healthy Choice Deadlines

March 31, 2018: Deadline to start participating to be eligible for full (6 month) Healthy Choice requirements.  The deadline for appeals of condition diagnosis or premium level is March 31st of each year.

June 30, 2018:  Deadline to start participating to be eligible for partial (3 month) Healthy Choice requirements.

Sept. 30, 2018: Deadline for meeting Healthy Choice requirements; health visit report form and participation data must be submitted


Privacy in Healthy Choice Program

Under HIPAA, EHP like other health insurers, is permitted to access health data for the purposes of claims payment, health program development and treatment coverage. The information sent to EHP member employees about next steps to participate in Healthy Choice are confidential communications between EHP and its members and contained EHP data generated using claims payment. wellness program and limited medical information. As with any of our healthcare plans and programs, plan member privacy is protected in full compliance with HIPAA.
